Help Analyzing This Game?

I really feel I got lucky pulling this win out of the fire, I thought for sure I was going to lose this game... and could use some help figuring out what serious blunders I made.

The moves I'm proud of:

Bishop exchange at move 17 allowed me to get my queen into position, and maneuvering my knight to back her up plotted out perfectly,
at moves: 21, 23 making room by pushing out the bishop.

12. Qe5+.  Then eat his d pawn.   Given your opponent's predilection for not-quite-immediately-suicidal attack, maybe it continues 12 ... Qe6 13. Qxd4 Rd8 14. Qe3 (something) 14. O-O and you're up a pawn, another of his is hopelessly isolated, your king is tucked away, and your development is ahead.

[I think -- sort of new at this, could be missing something stupid.  But if I'm not, that would have gotten you a free pawn and probably saved you a lot of running around the board keeping your queen out of trouble while he developed and you didn't (even if he did hand you the pawn on a silver platter later on, regardless).]

12.Qxd7 Kxd7 13.Bxd5 wins a pawn. (12.Qe5+ does as well, but with 12.Qxd7 you're also disabling his castling rights.) 

13.Qf4 doesn't threaten anything, it instead removes the threat of Qxg7, freeing the bishops duty of protecting it. Better is 13.Qe5+, which still wins a pawn.

24...Qc7 is horrible, 24...Be7 would've offered much more resistance.

Black overextended his centre and you made fine use of it, but you could've done this at move 12 instead of move 15.

Other than that, I don't see any point where you were in big trouble, at what point did you think you were losing control?

Oh!  Wow, maybe I simply got a bit worried when he was pointing his Queen and Bishop at my Kingside, I did cover it with my Queen though, but thought perhaps I had missed something else, thanks!
