It looks like you lost after you had achieved a crushing position.
While you were easily winning, your method of winning was way too slow while there were still queens, rooks and knights on the board.
You played very well the first part of the game, with good positional play and táctica, but when you were winning you misplayed. Rd3, as the previus comment say, was better, g4 looks ugly. Why to open your king's position? You should incorporate all your pieces in the attack! Cooperation is the key word! Then, you had a Rh3# move that you missed, and the ending was worse afterwards.
Wow! I feel so dumb missing that winning move.
