My Losses in Standard (Please analyze)

Minor things:
- 4. d4 is more accurate than Nf3, forcing Black to play ...d6 if Black wants to push the knight away from e5.
- You were right to avoid 8. Nc3 followed by dxc3. Normally, the big upside of having that structure is that the pawn on c3 gives added protection to White's king and makes pawn storms less effective. Neither is relevant with White castled kingside.
- In the variation where White plays g4 and takes on g6 with the queen, don't miss that Black's queen covers g6!
- ...Qe7 looks quite suspicious in that it loses time. If you get a normal Petroff position with extra tempi, that should be enough to consider ...Qe7 as "refuted". In the game, you got such a normal position with the queen on d6 rather than on d8, which shouldn't be a problem for Black.
Not-so-minor things:
- I don't like ...Bg4 followed by not taking on f3. Unquestionably 16. Ne5 is the best move. Then 16...Be6 17. Bc2 is good for White as you point out. If 16...Bh5 I like 17. Bf4 with a strong initiative. Besides the discoveries against the queen, Black also needs to look out for Qh3 followed by Nd7 and some ideas to win the pawn on b7.
- Don't trade queens! I don't like Qxg6 or Ne5. Qd1 or Qe2 allow Bxf3, so I guess Qd2 is the only move, then you can play Ne5 and get back on track. Once the queens come off, Black is better.
- Even if you had played dxe5 rather than hanging a piece, Black can play ...Bf5 Bxf5 Rxd2 at the end of that line with a big advantage.
When you have a bad pawn structure, you need activity to compensate for it. I'm sure you've heard that 791381234 times already and will hear it another 87623576415753672 times. Trading the queens gets rid of your activity, and I'm not sure you took that well enough into consideration.
I plan to post some games that I've lost (future losses also) in standard/rapid (most likely from chess.com) for analysis.
I would like to know what I could have done better and to improve.
