Who is supposed to initiate the handshake after the game the winner or loser what about a draw?

I don't think it matters.  Its simply a gesture of good sportsmanship.

Okay what about mate or stalemate on the board? I've just always assumed the loser does but could be wrong?

Whilst the handshake is almost universal, in Bulgaria it is customary to avoid eye contact and pretend to be doing something else before the game starts instead.
