Ouch, how dumb am I????

I was playing a friendly game with a 1560. The game was 30/10 and it was a closed game. It was a dead draw until I screwed it up. Darn! Any advice on how to have avoided these errors? Thanks. You can check out the game here: http://www.chess.com/livechess/game?id=616459480

play through a grandmaster(or anyone, it doesn't even have to be a master) game and look at all the threats for each move. when you are playing think "what are his possible captures, checks, and threats" before each move.
