The obvious answer is "Accuracy is a joke." A very bad joke.

Seriously, both of these players' accuracy was 99.4% in this game (false obviously)
Accuracy could be inflated by long and correct endgames I guess

I just lost two games as a beginner rated around 400 here. I lost with one mistake and one blunder to an instant checkmate by queen on move 21. Accuracy was 27%. I played much the same game losing with the same number of mistakes and blunders around the same point but scored 92% accuracy. The difference? The first was a shock loss in 21. The second I saw coming after my blunder and managed to make the checkmate difficult and danced my king for 37 more moves. I wouldn’t call it an endgame as in lots of pawn advances and rooks chasing rooks. I just danced to avoid a difficult checkmate for a long time. I would have resigned but the bots don’t care. It was 37 accurate accurate moves but almost all of them were obvious or forced. So take accuracy for what it is worth.

Believe it or not, even chess.com thinks CAPS is a joke. There is an option chess.com provides called ,"max CAPS", that gives you a more accurate score. It takes 3 ~ 5 minutes, but solves many of the problems that people are complaining about CAPS.
If you are unwilling to spend 3 ~ 5 minutes calculating your accuracy, you really shouldn't complain.

Are commas a joke? Also CAPS score is inaccurate. Maybe they want to encourage you or something, like “Oh you still played well even with these mistakes!”

Yes, I was confused by a "blunder" that led to a checkmate shortly thereafter, but there was probably a better way to win that didn't involve losing a piece.

I just noticed whenever I lose a game, my accuracy score is horrible like 20% but if I win it is in the 90's even though, there is not a lot of differences in terms of mistakes and inaccuracies.

The concern is - is the tool bugged?
The final report gives a total for best moves and a total for excellent moves played.
The final analysis when viewed states a move was the best move or an excellent one. When they are added in the analysis -
The report and the analysis give different totals.
For the example game - a discrepancy of 4 is substantial. It’s not related to depth of search but certainly effects an accuracy score .
