A rollercoastergame (online)

For a tournament I played an online game. It was a good game (to me) in which we where both moving very precisely. Me trying to break through his defense, but he saved nicely every time. Then I made two 'misclicks'. I thought I was working in Analyze but in fact I was working in the real game.. somehow I eventually took the win!? This was a rollercoaster to me: I thought I was going good, then I was sure it was going to be a drawm then I thought I would lose, but eventually took the win!

A couple questions:
- did I have a possibility to break through?
(from move 16. the kingside was the front and I hoped to win material or to make the h-pawn dangerous)

- did he have an oppurtunity to mate me when I was attacking? 
( I feared 34. ... Qg1+ -eternal check? or even a forced mate if the king went to a3 or c3)

- how could he have prevented me from winning?
(after 52. Kc4?? I thought I was dun goofed.. how could black take the win from there? Working towards a K+R vs K endgame?)
