How did I get into such a bad position?

I rarely post games that I won, but I won only because my opponent made a blunder. I had a terrible position with almost no counterplay. When I went over the game, I couldn't identify any glaring mistakes, but I must have made some. I would like to know where I went wrong. This was an OTB game, time control 110+30. (Time pressure was not a factor.) I have not done the computer analysis.
