your notation is messed up at move 30 ... Cxc5, and it would be helpful if you inserted this as a game board instead of all notation.
My best game so far

9. Bd5 probably not good as you trade a bishop for a knight, and mess up your pawn structure in the exchange.
13. h3 not a good move since after 13 ...Bxf3 14. gxf3 your pawn structure is more borked and your king is on an open file, inviting an assault from the black queen. Better would have been 13. Qe3, better still would have been not to put your queen on d2 to start with, so that your knight had somewhere to run to.
After 14. ...Bh4 its quite clear that Black is about to bring the pain on the kingside. You should have figured out a way to defend or run, instead of the queenside development imo.
19. Bb2 is also confusing, leaving your knight hanging.
The rest of the moves are a result of 13. h3.
