I had a relative in the hospital recently. Hospital food is some of the worst food on the planet...
Loved hospital food when I was there. I was eight though, and I was willing to eat anything but rice which may have influenced my opinion

I am getting ready to be a nutrisionist and from my opinion, the bad stuff in a Big Mac is the fat (saturated and trans) and also the white bread that gets stuck in your stomach and takes a lot of time to get out of the body so it's also a problem. And the vegetables are probably converted somehow (added bad nutrients to it)
Well, there shouldn't be any trans fat in anything, although the food manufacturers can use a loophole where if the serving size is small enough, and if the trans fat is less than 1/2 of a gram per serving, then they can say it's zero. You have to look at the ingredients and see if there is anything that's hydrogenated or partially-hydrogenated. But yeah, trans fat is the absolute worst thing you can put in food. It's poison. Your body can't break it down, and it just lingers in your system and causes problems.
Saturated fat is okay. Yeah, I know, everybody says it's bad, but they're wrong. Do your research and you'll find out. (And don't read those epidemiological studies - Read the randomized control trials.)
The other bad stuff is the sugar and highly processed carbs, which includes the buns.
