Apparently many people might not realize the Queen is hanging because they don't realize that the white pawn is coming down the board (look at the coordinates as the puzzle is correctly oriented from the black pieces perspective as it is black to move). I would guess this puzzle rating is inflated because many people try something like ...Qf8+?? instead.
