Yes it has to go to the second rank for a bongcloud and it has to be within the first 7 moves. The magnus carlsen variant, therefore, is not a bongcloud

I will be the one to answer this question out of the fear of incorrect information being spread. If the king and queen switch places through any means in the opening, then it is considered to be within the realm of the Bongcloud whether or not the king moved forward.
