I've had this happen several times, but this one was very clear as it was a blitz and not a one minute bullet. Still I was in quite big time troubles, but luckily I had mate.

I got the queens on the d- and e-files. And premoved them going two ranks towards the king each, so three premoves to mate, not much one could do wrong here. However instead of the d-queen going to d7, the e-queen was moved on move 49.
Luckily there was still time to save it, but it could have been very frustrating to lose in that situation.
It seems that when two of the same pieces can go to the same square, the premoves mess it up sometimes and move the wrong piece. I'm very certain I premoved the sequence right, they were still marked red, but I got suspicious when I didn't hear the mate sound and there have been threads about the wrong rook moving in similar circumstances.
